New Patients
Common Questions About Our Appointments
Please fill out this new patient form, and our new patient representative will contact you via phone and text. The scheduling process is very simple and takes about 5–10 minutes. The new patient representative will determine the length of your first appointment based on your health goals and medical history and provide you with a transparent pricing breakdown. Afterwards, we usually review office policies and afterwards our staff will send you email links to complete your set of new patient questionnaires before your appointment, and submit a deposit to hold your appointment time.
YOU DO NOT need to get labs done before your appointment, as our doctors will most likely order customized lab work based on your initial evaluation.
A treatment plan based on your symptoms that will be adjusted once your lab work results come in and will be tailored to your medical history.
- Thorough lab work is typically sent to Quest or LabCorp, depending on your evaluation. Our staff will also make recommendations for Specialty Lab Work after the initial consultation as needed to evaluate your medical condition.
- Your Supplements will be sent to you via our online dispensary and your prescriptions will be sent to your pharmacy.
- A list of medical-grade supplements emailed to you for purchase on Fullscript.
- Follow-up appointments are available at an additional cost to track your progress.
We completely understand how infuriating the modern-day healthcare system can be, everything from doctors who don’t listen to you when you say you’re not feeling well, to 15-minute appointments that leave you feeling unheard while not addressing the root cause. If you want to get a feel for our doctors, you are welcome to request a FREE 10-minute discovery call to see if our doctors seem likely to meet your needs. This can be scheduled with our new patient care
coordinator after filling out our new patient inquiry form. After the call, there is NO OBLIGATION to schedule if you are not comfortable, and you will have a better idea of how our physicians can support you on your healing journey.
We are classified as an out-of-network provider and do not directly deal with insurance. We have chosen this model because insurance limits our ability to tailor our protocols to our patients’ needs. Our focus is to provide a patient centered approach to care and our emphasis relies on putting the patient first in all circumstances. After your appointment, we will provide you with an invoice with codes so you can individually submit it to your insurance to see if they will cover any costs. However, we do NOT guarantee that insurance will cover our appointments or treatments—this depends on your out-of-network benefits and your insurance provider.
To check your benefits, you may want to contact your insurance provider and ask:
- Does my plan offer out-of-network reimbursement for medical visits?
- What percentage of the visit cost will be reimbursed?
- Do I need pre-authorization or a referral for reimbursement?
We do accept CareCredit. We also accept FSA cards.

Office Policies:
Intake and Follow-up Visits
Normally, our intake new patient first visit is 1-2 hours long. This is all time you will get one-on-one with the doctor, where she will review symptoms of your various systems, and review any relevant environmental history. The length of the visit depends mainly on the complexity of the symptoms, the age of the patient, and the length of time the patient has had symptoms.
Follow up visits can be done as office visits. We also offer phone consults if reaching the office is logistically difficult. The length of the appointment depends on the individual; their complexity of treatment, age, progress, and place of residence. Duration of these visits varies between 30, 45, and 60 minutes. We understand some patient may prefer phone consultations, especially finding it difficult to travel while being sick, however, we do need to see you for an in-office visit at least once every 6 months to be able to evaluate you and adjust your treatment plan when needed.
We are unable to send in blood work orders or request tests before your first intake visit. Dr. Afrooz prefers to evaluate to see you first, evaluate your symptoms, do the physical exam, so she is able to better determine what laboratory tests need to be ordered.
Once you have made the decision to become our patient, we would get you set up with a patient portal, where you can communicate with the doctors and the office staff, update your demographic information, complete intake questionnaires, and if you already have relevant lab results and medical records, you can upload them to your portal or bring a copy with you to your visit.
The length of the treatment, the frequency of follow-up visits, and the nature of a treatment plan varies based on the patient’s severity and duration of symptoms, test results, age, among many other factors. Therefore, we will be able to give you more answers to these questions after the doctor evaluates you.

We are an insurance free practice and classified as an out-of-network provider. We do not file for insurance reimbursement. However, we will provide an invoice you can submit to your insurance, to help you get as much of your visit covered. Unfortunately, we cannot guarantee your insurance would reimburse you for your visits. We do offer payment plans if needed.
Fees and Charges
When scheduling your first visit, we require a $100 deposit to hold your appointment. This goes towards your first visit, and is fully refundable if appointments are canceled or rescheduled 3 business days prior to the visit.

Elle2025-02-25I've been going to Dr. Afrooz since 2021 and before she switched buildings to this one. She gets to know her patients well and always knows what to look for and test for. She's been so vital in my quality of life and has helped me and my family every step of the way. Even when I was at my darkest, she was there helping from the sidelines. My entire family has all been helped by Dr. Afrooz and her husband. We've made connections and friendships and became familiar with each other in a way that other doctors are usually more one-dimensional with. Dr. Afrooz has changed my life in such a wonderful way! She is worth every penny we've paid and way more!Trustindex verifies that the original source of the review is Google.
Manzur Ahmed2024-07-05Dr Afrooz is my favorite. She is very knowledgeable and patient with her patients. I was always feeling very tired and lethargic and I was thinking that maybe I had long covid. But Dr Afrooz knew all the right questions to help identify that my condition is related to tick bites instead from a hiking trip I had gone on. I ask a lot of questions and she seems to not find me to be frustrating and i know that she is straightforward because she’s not afraid to say I don’t know if she does not know the answer to any of my questions. Nor is she dismissive of me whether it be ideas as to what I think could possibly help me or if I tell her something does not feel right. I highly recommend her and I feel fortunate to have been able to see her.Trustindex verifies that the original source of the review is Google.
Muhammad Haroon2024-06-27This is one of the most convenient doctor offices that I have been to. The appointments and treatments here are not nearly as atrociously priced as most of my visits to other doctors. The staff and workers really make me feel cared for. I recomend this doctor office over many other ones.Trustindex verifies that the original source of the review is Google.

Oxana Oleson2024-06-26I have known Dr. Afrooz for the past 8 years. Besides her knowledge and care she put in helping patients with different complex health issues, she is also one of the nicest people I know. She treated me and my family and never gave up on me trying to find a solution for treating chronic Lyme disease. Her office is equipped with lots of helpful tools to get you well. My favorite is a hyperbaric chamber, but there are many more. My daughter also recovered from Lyme disease with help of Dr. Afrooz. At that time, she was only 5 years old. I am beyond grateful for her help and knowledge. She is using both-conventional and integrative methods and always keeping up to date on new treatments. ,Trustindex verifies that the original source of the review is Google.
